HUBUNGAN ANTARA KADAR 8-HYDROXY-2-DEOXYGUANOSINE SERUM DENGAN FUNGSI KOGNITIF PADA STROKE ISKEMIK

CORRELATION OF SERUM 8-HYDROXY-2-DEOXYGUANOSINE CONCENTRATION WITH COGNITIVE FUNCTION IN ISCHEMIC STROKE

ABSTRACT

Introduction
: Post-stroke disabilities attributed to physical disorders can be worsened by the presence of cognitive dysfunction. Serum 8-hydroxy-2-deoxyguanosine (8-OHdG) is the result of free radical interaction with the C8 group of guanine base. Increased serum 8-OHdG concentration indicates neuronal damage, and affects cognitive function in ischemic stroke patients, examined usingMontreal Cognitive Assessment–Indonesia version (MoCA-Ina).


Aims:
To determine the correlation of serum 8-OHdG concentration with the decrease in cognitive function in ischemic stroke patients.


Method:
This was a cross-sectional observational study in the acute phase of ischemic stroke patients in Dr. Hasan Sadikin Hospital, Bandung, in September 2014 to January 2015. The serum 8-OHdG concentration was measured and cognitive function was assessed using MoCA-Ina. Statistical analysis was performed using non-parametric Kruskal-Wallis test and rank Spearman's correlation test.


Results:
There were 44 subjects; most were female (66%), within 50-59 years age group (45.4%), and with elementary level of education (72%). ROC curve analysis revealed that the cut off point of serum 8-OHdG levels was 5.7ng/ml. Any levels above 5.7ng/ml showed cognitive dysfunction based on abnormal MoCA-Ina (90.9%). The correlation between increased levels of serum 8-OHdG anddecreased cognitive function in acute ischemic stroke was very significant (rs=-0.807, p<0.001).


Discussion
: This study showed strong significant negative correlation between serum 8-OHdG concentrations and cognitive dysfunction assessed using MOCA-Ina in acute ischemic stroke patients.

ABSTRAK

Pendahuluan
: Disabilitas pasca stroke selain gangguan fisik diperburuk dengan adanya gangguan fungsi kognitif. Senyawa 8-hydroxy-2-deoxyguanosine (8-OHdG) merupakan hasil interaksi radikal bebas dengan gugus C8 basa guanin. Peningkatan kadar 8-OHdG serum menandakan kerusakan sel neuron di otak dan memengaruhi penurunan fungsi kognitif penderita stroke iskemik yang diperiksa dengan menggunakan instrument Montreal Cognitive Assessment–versi Indonesia (MoCA-Ina).

Tujuan: Mengetahui hubungan peningkatan kadar 8-OHdG serum dengan penurunan fungsi kognitif menggunakan MoCA-Ina pada stroke iskemik akut.

Metode: Penelitian observasional dengan studi potong lintang pada pasien stroke iskemik akut di RSUP Dr. Hasan Sadikin pada bulan September 2014 hingga Januari 2015. Dilakukan pengukuran kadar 8 OHdG serum dan fungsi kognitif menggunakan MoCA-Ina. Analisis statistic menggunakan uji non parametrik Kruskal-Wallis dan uji korelasi rank Spearman’s.

Hasil: Pada penelitian ini didapatkan 44 subjek, terbanyak perempuan (66%) dan kelompok usia 50-59 tahun (45,4%). Berdasarkan kurva ROC (receiver operating characteristic) diperoleh cut off point kadar 8-OHdG serum 5,7ng/ml. Peningkatan kadar 8-OHdG menunjukkan gangguan fungsi kognitif (90,9%) yang sangat bermakna (p<0,001, rs=-0,807).

Diskusi: Terdapat korelasi negatif sangat kuat dan bermakna antara peningkatan kadar 8-OHdG serum dengan penurunan fungsi kognitif berdasarkan MoCA-Ina pada pasien stroke iskemik akut.
