HUBUNGAN KADAR SERUM BRAIN-DERIVED NEUROTROPHIC FACTOR DENGAN GANGGUAN KOGNITIF PASCACEDERA KEPALA

THE RELATIONSHIP BETWEEN SERUM BRAIN-DERIVED NEUROTROPHIC FACTOR LEVEL AND COGNITIVE DYSFUNCTION AFTER HEAD INJURY

ABSTRACT

Introduction: Cognitive dysfunction is a chronic complication of head injury associated with the ability to regenerate, cell plasticity, and learning proses. Brain-derived neurotrophic factor (BDNF) is one of the neurotrophin, playing a role in synaptic plasticity, and learning process. Research on impaired cognitive function and neurogenesis response after acute head injuries is rare.

Aims: To determine the relationship between serum BDNF levels at head injury and cognitive dysfunction after the head injury.


Methods: This was an observational cohort study with 30 subjects with head injury in Dr. M. Djamil Central Hospital, Padang from November 2014 to June 2015. Serum BDNF level was analyzed using enzyme-linked immunosorbent assay (ELISA). Cognitive function was calculated with Montreal Cognitive Assessment Indonesian-version (MOCA-Ina). The relationship between serum BDNF levels with cognitive function at admission was analyzed using Chi-square. The correlation of serum BDNF levels one month after onset and factors affecting cognitive function after injury was analyzed using Pearson and Spearman.


Results: The means of serum BDNF levels at admission and after one month were 576.85±306.65pg/mL and 897.46±344.92pg/mL, respectively. There was a significant difference in BDNF levels at admission and one month after injury. There was a significant correlation between decreasing serum BDNF levels at admission with cognitive dysfunction. There was significant correlation between serum BDNF level after one month, age, GCS score, and educational background with impaired cognitive function after head injury.


Discussion: There was a significant association between decreasing serum BDNF level at onset of head injury with impaired cognitive function after a head injury.

ABSTRAK 

Pendahuluan: Gangguan fungsi kognitif merupakan sekuele cedera kepala yang bersifat kronis berhubungan dengan kemampuan regenerasi, plastisitas sel, dan proses belajar. Brain-derived neurotrophic factor (BDNF) merupakan suatu faktor neurotropik yang berperan penting dalam plastisitas dan proses belajar. Penelitian tentang gangguan kognitif dan respons neurogenesis pascacedera kepala belum banyak dilakukan.

Tujuan:
Mengetahui hubungan kadar serum BDNF dengan gangguan fungsi kognitif pascacedera kepala.

Metode:
Penelitian ini menggunakan disain observasional kohort pada pasien cedera kepala di RSUP Dr. M. Djamil dari November 2014 sampai Juni 2015. Dilakukan pemeriksaan kadar BDNF serum saat awal masuk dan satu bulan kemudian diulang bersamaan dengan penilaian kognitif menggunakan Montreal Cognitive Assesment versi Indonesia (MoCA Ina). Hubungan kadar serum BDNF saat masuk dan fungsi kognitif dianalisis dengan uji Chi-square sedangkan hubungan kadar serum BDNF satu bulan onset dan faktor yang mempengaruhi kognitif pascacedera kepala dengan uji Pearson dan Spearman.

Hasil:
Didapatkan 30 subjek dengan rerata kadar serum BDNF saat masuk 576,85±306,65pg/mL yang meningkat bermakna satu bulan kemudian 897,46±344,92pg/mL. Terdapat juga hubungan bermakna antara penurunan kadar serum BDNF saat masuk dengan gangguan fungsi kognitif, serta antara kadar BDNF satu bulan onset, usia, nilai Glasgow Coma Scale (GCS), dan lama pendidikan dengan gangguan fungsi kognitif pascacedera kepala.

Diskusi:
Penurunan kadar serum BDNF berhubungan bermakna dengan gangguan fungsi kognitif pascacedera kepala.
